Objective: Birthing people who experience pregnancy complications are at a higher risk of developing perinatal anxiety or depression. Antepartum hospitalization due to these complications enhances the risk, with one in every three birthing people hospitalized reporting anxiety or depression. Our objective was to evaluate interventions for anxiety or depression for birthing people experiencing antepartum hospitalization for perinatal or fetal complications.

Study Design: Searches were conducted in Ovid MEDLINE, Embase, CINAHL Plus, Cochrane CENTRAL, and PsycINFO to identify randomized controlled trials (RCTs) comparing any intervention against treatment as usual (TAU) for management of anxiety or depression among birthing people admitted to a hospital’s antepartum unit for pregnancy complications. Two authors independently reviewed abstracts for eligibility and potentially eligible full-text articles for inclusion. The primary outcome was a measurement of anxiety or depression symptoms using a validated scale following the intervention. The Hedges method was used to detect standardized mean difference (SMD) in studies using different psychometric scales.

Results: Of 877 articles identified, two RCTs—consisting of total of 161 and 123 participants randomized to intervention and TAU, respectively—were included. The pooled intervention group included daily meditation programs or other mind-body interventions such as education on stress reduction techniques and individualized patient support. Compared to TAU, interventions significantly reduced mean scores assessing anxiety and depression symptoms (Figure 1).

Conclusion: Few RCTs have been published examining the effect of interventions on symptoms of anxiety or depression among birthing people who experience antepartum hospitalization. Nevertheless, mental health interventions yielded modest yet significant reduction in both perinatal anxiety and depression symptoms in this population. These promising results and shortage of data highlight a need for further high-quality trials to support the mental health needs of this high-risk population.
