Objective: To compare mean gestational ages at delivery between different interventions after early identification of short cervical length

Study Design: Retrospective cohort analysis of pregnant women who were identified with a short cervix at an initial 16 week to 22 week ultrasound at a teaching community hospital were performed from January 2016 to December 2021. All patients with a short cervix defined as 25mm or less were offered treatment with vaginal progesterone, cerclage, 17-hydroxyprogesterone (17-OHP), or with a combination of the therapies.

Results: A total of 293 pregnant patients were identified and evaluated for having a short cervix as defined by less than 25mm. They received either vaginal progesterone, cerclage, vaginal progesterone and cerclage, 17 OHP, or cerclage with 17 OHP for prevention of preterm birth in their pregnancies. In the less than 15mm cervical length group, there was no statistically significant difference in the gestational age of delivery (F value 1.49, p value: 0.19). In the less than 15-20mm, cervical length group, there was no statistically significant difference in the means of all treatment groups (F value 0.85, p value: 0.53). And in the 20-25mm cervical length group, there was no statistically significant difference in the means of all treatment groups (F value 0.56, p value: 0.76).

Conclusion: Our results show there is no significant difference in mean gestational age at delivery for women who received a cerclage, vaginal progesterone, 17-OHP, or a combination of the treatments after short cervix identification at 16-18 weeks.
