(744) Visual Aids for Induction of Labor Education: A Randomized Controlled Trial

Objective: There are racial disparities in birth satisfaction among those undergoing induction of labor (IOL). Our objective was to develop a visual aid informed by the experience of historically marginalized pregnant people and test its ability to meet their IOL education needs. 

Study Design: This was a randomized trial of patients admitted for IOL. Those with prior cesareans, fetal anomalies, fetal growth restriction, and non-English speakers were excluded. Using data from an initial qualitative phase of semi-structured interviews and surveys of patients from marginalized backgrounds, we developed a visual aid handout on induction methods. On admission for IOL and prior to counseling on type of initial IOL method, patients were randomized to routine counseling or counseling with the visual aid. The primary outcome was patient knowledge, using a published IOL knowledge assessment tool. Secondary outcomes were Trust in Physician Scale scores and satisfaction with IOL counseling via Likert scale. A stratified analysis was performed by self-reported race (Black/Latinx/mixed race vs white). Eighty-four patients were required to detect a 25% difference in knowledge scores with 90% power. Analysis was performed using intention to treat approach.   

Results: Eighty-four p</span>atients were randomized: 42 to routine counseling, 42 to visual aid. 31% of patients self-identified as Black, Latinx, or mixed race. There was no significant difference in knowledge score between routine counseling and visual aid groups (74.8% vs 68.3%, p=0.17). Trust in physician scores and satisfaction scores were also similar between groups. Although the visual aid increased knowledge scores in Black/Latinx/mixed race patients by 16%, there was no significant difference between routine counseling and visual aid groups.

Conclusion: The addition of a patient-informed visual aid to routine counseling at time of IOL did not significantly increase patient knowledge, trust in physician, or satisfaction scores. This calls for a broader investigation into contributors of birth dissatisfaction and consideration of earlier interventions prior to start of IOL.
