(437) Differences in Gestational Age at Preeclampsia Diagnosis and Delivery Across BMI Categories

Objective: Our objective was to determine if body mass index (BMI) category is associated with gestational age at time of diagnosis of preeclampsia-spectrum disorders, gestational age at time of delivery, and/or the latency period between the two. Obesity is associated with earlier onset of preeclampsia and shorter latency from diagnosis to delivery.

Study Design: A single center, retrospective, cohort study was conducted of all deliveries that occurred at Loma Linda University Medical Center in Loma Linda, California in 2019. Patients with a diagnosis of preeclampsia were categorized into Underweight / Normal Weight (BMI ≤ 24.9 kg/m²), Overweight (BMI 25-29.9 kg/m²), Class I Obesity (BMI 30-34.9 kg/m²), Class II Obesity (BMI 35-39.9 kg/m²), and Class III Obesity (BMI ≥ 40 kg/m²). Median number of gestational age at time of preeclampsia diagnosis, gestational age at time of delivery, and the latency period were compared across BMI categories. Completed weeks of gestation was used for gestational age.

Results: For all patients with a diagnosis of preeclampsia, BMI category was significantly associated with gestational age at time of diagnosis (p = 0.0017) and gestational age at time of delivery (p = 0.0038). BMI category was not associated with the latency period between the two (p = 0.46).

Conclusion:

In our population, BMI category is significantly associated with gestational age at time of preeclampsia diagnosis and gestational age at time of delivery but not associated with the length of latency period between them. Contrary to our expectation, we observed that lower BMI was associated with earlier diagnosis and delivery.
