Objective: Abnormal maternal serum analytes measured during prenatal screening have been associated with adverse pregnancy outcomes. We sought to examine the association between first and second trimester maternal serum analyte levels obtained for prenatal aneuploidy screening and stillbirth (fetal loss >20 weeks gestation)

Study Design: Retrospective cohort study of all singleton, nonanomalous pregnancies with no known genetic disorders dated by first-trimester ultrasound with available serum analyte data who delivered between from 3/1/2017 to 12/31/2022. Multiple of the median (MoM) values for free beta-human chorionic gonadotropin (β-hCG), pregnancy-associated plasma protein A (PAPP-A), maternal serum alpha-fetoprotein (AFP), unconjugated estriol, and inhibin A) were compared between two groups: patients with stillbirths (i.e. study group) and those with live births (i.e. control group) using variable analyses. Standard cutoffs for abnormal analyte levels were also compared between the two groups. Multivariable analyses were performed to adjust for potential confounders.

Results: Of the 5,092 pregnancies included in the study, 45 (1.1%) had a stillbirth. After adjusting for potential confounders, there were no statistically significant differences in mean serum analyte levels between the two groups (Table 1). Additionally, the proportions of abnormal analyte levels were similar between the two groups.

Conclusion: The results of this large cohort demonstrated no association between serum analyte levels and stillbirth. Our data does not support use of these biochemical markers as a screening tool for stillbirth.
