(133) Anemia negatively impacts perinatal outcomes in a large cohort of 1,724,828 pregnancies in California.

Objective: Anemia is common in pregnancy and associated with maternal and fetal morbidity. However, the incidence and risk factors of perinatal outcomes are largely derived from international cohorts with higher rates of anemia and are less representative of pregnancies within the United States (US). The objective of this study was to evaluate perinatal outcomes associated with anemia diagnosis in pregnancy in a modern, large California-based cohort.

Study Design: We performed a cohort study of California-linked hospital discharge-vital statistics data from 2016-2019. Deliveries 23-42 weeks’ gestation were included and anemia was identified using birth certificate and International Classification of Disease (ICD-10) codes. Chi-squared and multivariable logistic regression were utilized for statistical analyses.

Results: A total of 1,724,828 births met study inclusion criteria; 303,718 (18%) had a maternal diagnosis of anemia. Pregnancies affected by anemia compared to those without anemia were associated with a higher frequency of hypertensive disease (12.1% vs 8.1%, aOR: 1.51 (1.49-1.53)), preterm delivery (10.4% vs 7.7%, aOR: 1.30 (1.28-1.32)), and very preterm delivery (1.8% vs 1.1%, aOR: 1.49 (CI: 1.45-1.54). Neonatal morbidity was increased in the presence of maternal anemia with more NICU admissions (11.5% vs 9.5%, aOR: 1.12 (1.11-1.14)), respiratory distress syndrome (7.5% vs 5.6%, aOR: 1.33, (1.31-1.35)), hypoglycemia (3.8% vs 2.8%, aOR 1.34 (1.31-1.37)) and low birth weight infants (< 2500 gm) (7.9% vs 6.1%, aOR 1.22 (1.20-1.24)).

Conclusion: Anemia during pregnancy is associated with numerous adverse perinatal outcomes. These findings support anemia as a potentially modifiable risk factor in maternal and neonatal morbidity. Increased screening for anemia may provide timely intervention and mitigation of associated morbidity
