Objective: To identify socioeconomic factors associated with self-reported physical activity during pregnancy and its impact on composite maternal and neonatal outcomes through a large prospective study

Study Design: This is a secondary analysis of the Nulliparous Pregnancy Outcome Study: Monitoring Mothers-To-Be (nuMoM2b). All subjects participating in the nuMoM2b study were surveyed on whether they were physically active at three separate prenatal visits. Subjects were eligible for this analysis if they responded to this question at all three visits. Those that answered “Yes” at all visits were grouped in the “Consistent Activity” category while the remainder were grouped in the “Limited Activity” category. Delivery complications were grouped into composite maternal morbidity and composite neonatal morbidity outcomes. Statistical analysis included Chi-square, student t-tests, and multivariate logistic regression models.

Results: From the 10,038 subjects enrolled in the NuMoM2b study, 9,057 (90.2%) were eligible for this analysis. Of these subjects, 4,734 (52.3%) were in the “Consistent” group while the remaining 4,323 (47.7%) were in the “Limited” group. Subjects in the “Limited” group were more likely to be from a non-white background (48.3% vs 28.8%, P< 0.0001), have government insurance (34.2% vs 17.1%, P< 0.0001), be below the poverty line (16.4% vs 9.0%, P< 0.0001), and have at most a high school level education (27.0% vs 11.2%, P< 0.0001). The “Consistent” group had a lower proportion of both composite maternal morbidity (44.3% vs 46.8%, P = 0.019) and composite neonatal morbidity (15.6% vs 17.5%, P = 0.011). In multivariate logistic modeling, consistent physical activity remained an independent predictor of composite maternal morbidity but not composite neonatal morbidity.

Conclusion: Subjects from socially or demographically disadvantaged groups are more likely to report limited physical activity in pregnancy. Consistent physical activity throughout pregnancy was associated with fewer adverse maternal outcomes but no difference in adverse neonatal outcomes.
