(799) Prophylactic Aspirin Treatment for Preeclampsia Prevention in Dichorionic-Diamniotic Twin Pregnancies

Objective: Preeclampsia (PE) is a pregnancy-related hypertensive disorder associated with significant maternal and neonatal morbidity and mortality. Definitive treatment requires delivery, hence primary prevention is crucial. Aspirin has shown success in preventing early PE in singleton pregnancies. However, its efficacy in twin gestation is unknown.
We aimed to evaluate whether prophylactic use of aspirin initiated prior to 16 weeks gestation reduces PE incidence in dichorionic (DC) pregnancies

Study Design: Retrospective cohort study of 1,008 DC pregnancies between 2015 and-2022 in a single tertiary center, of whom 180 (90 treated vs 90 untreated) were compered using propensity score matching to estimate the effect of aspirin treatment on PE occurrence. The propensity scores were estimated using logistic regression based on maternal age, BMI, chronic hypertension, antiphospholipid antibodies syndrome (APLA), parity, in vitro fertilization (IVF), smoking status, history of PE, history of preterm delivery and history of gestational diabetes

Results: We found no significant difference between the groups in the rate of PE with or without severe features (8.9% vs 12% p=0.467 ;8.9% vs 4.4%, p=0.232), respectively. Furthermore, the groups did not differ in the incidence of gestational hypertension, superimposed PE, HELLP syndrome, gestational diabetes, and fetal growth restriction. When comparing gestational age at PE diagnosis, no difference was found between the groups  before and after 37 weeks gestation (16% vs 22%, p=0.253; 2.2% vs 1.1%, p >0.999), respectively (Table 1). Further logistic regression analysis failed to demonstrate that aspirin treatment reduces rates of any hypertensive disorders, PE with or without severe features (OR 1.496 95% CI 0.762 - 2.937; OR 0.701 95%CI 0.267 -1.841; OR 2.098 CI 95% 0.651 - 6.760), respectively, independent of the differences of gestational ages at birth. The need for induction due to PE did not differ between the groups (OR 0.551 CI 95% 0.169 - 1.801)

Conclusion: Administration of prophylactic aspirin in DC pregnancies did not result in a reduction of PE incidence
