(1112) Prevalence rates of chronic hypertension and hypertensive disorders of pregnancy: US national trends 2015-2020

Objective: Chronic hypertension (cHTN) and hypertensive disorders of pregnancy (HDP) represent one of the leading causes of severe maternal morbidity and mortality in the United States (US). However, data on the population burden of these disorders is lacking. We sought to quantify recent prevalence rates and trends of cHTN and HDP among pregnant patients.

Study Design: We performed a retrospective cohort study of all US pregnancies between 2015 to 2020 using the National Vital Statistics. Annual prevalence rates were calculated for cHTN and HDP and visualized graphically. Risk difference (RD), relative risk (RR), and their 95% confidence intervals (CI) comparing cHTN and HDP rates for 2020 with 2015 were estimated.

Results: A total of 23,027,245 births were included in the analysis. In the US, rates of cHTN increased from 1.65% (95% CI: 1.63%, 1.66%) in 2015 to 2.54% (2.52%, 2.56%) in 2020 with a statistically significant RR of 1.54 (95% CI: 1.52, 1.56) and an absolute RD of 0.89% (95% CI: 0.87%, 0.91%). During the same period, rates of HDP increased from 5.84% (95% CI: 5.81%, 5.86%) in 2015 to 8.65% (95% CI: 8.62%, 8.68%) in 2020 with a RR of 1.48 (1.47, 1.49) and an absolute RD of 2.81% (2.77%, 2.85%).

Conclusion: National prevalence of cHTN and HDP have increased in recent years. This trend underscores the need for targeted interventions to address perinatal cardiometabolic health. More studies are needed to explore the factors influencing the rise of hypertensive disorders nationally.
