(335) Efficacy of glycated albumin in the management of gestational diabetes

Objective: While glycated albumin (GA) can be used for blood glucose management in patients with diabetes, the role of GA in assessing glycemic status in gestational diabetes (GDM) has been controversial. This study aimed to investigate the efficacy of GA measurements in predicting the need for insulin therapy at GDM diagnosis and assessing glycemic status during the early third trimester in women with GDM.

Study Design: This retrospective study included singleton pregnant women with GDM who had both GA and self-monitoring of blood glucose (SMBG) measured at the time of diagnosis and between 31-33 weeks’ gestation. SMBG measurements were taken four times a day (before breakfast [fasting] and 2 hours after each meal), and the respective mean values of fasting and postprandial glucose measurements over consecutive seven days were calculated. We investigated whether GA was correlated with SMBG values and whether GA values at diagnosis and during the early third trimester effectively predicted insulin therapy and poor glycemic control (defined as fasting ≥95 mg/dL and postprandial ≥120 mg/dL, respectively.

Results: We measured GA in 190 patients at diagnosis and in 153 patients during the early third trimester. While GA values were significantly and negatively associated with prepregnancy body mass index (BMI) (p< .0001), they did not show significant associations with mean fasting or postprandial glycemic values of the SMBG measurements, either at diagnosis or during the early third trimester after controlling BMI. GA did not differ significantly between women with and without insulin therapy or those with poor and good control (Table).

Conclusion: GA measurements seem to be ineffective in assessing glycemic control status in women with GDM and therefore should be abandoned.
