(188) Adverse pregnancy outcomes associated with hepatitis C in pregnancy: a population-based study

Objective: The prevalence of hepatitis C virus (HCV) infection during pregnancy in the United States (US) continues to increase at an alarming rate. Data regarding the association of HCV with adverse pregnancy outcomes are limited. Therefore, we set out to evaluate adverse pregnancy outcomes associated with HCV during pregnancy in a current US population.

Study Design: Retrospective analysis of the US Centers for Disease Control and Prevention Natality Live Birth database (2016-2021). All live births were eligible for inclusion. Deliveries with missing data on HCV were excluded. The rates of several adverse pregnancy and neonatal outcomes were compared between two groups: pregnancies complicated by maternal HCV infection and those without HCV. Multivariable logistic regression was performed to adjust outcomes for potential confounders. Data were presented as adjusted odds ratios (aOR) with 95% confidence intervals (95% CI) and statistical significance set at P < 0.05.

Results: Of the 22,604,938 included, 107,761 (0.48%) were complicated by maternal HCV infection. Adverse pregnancy and neonatal outcomes associated with HCV during pregnancy are presented in the Table.

Conclusion: Based on this large US population cohort, HCV infection during pregnancy is associated with an increased risk of several adverse pregnancy outcomes and neonatal complications. Specifically, the risk of preterm birth, congenital malformations, low birthweight, NICU admission, low Apgar score, need for ventilation, and antibiotics for suspected neonatal sepsis were significantly higher in pregnancies complicated by HCV. Understanding the impact of HCV on pregnancy outcomes in a current US population is important to better manage and optimize care for these pregnancies.
